When you are registering or making an appointment to register for the testing, we will ask you to provide us with some prerequisite information. You may choose to provide personal information or any information that you may find necessary. Please note that if you fail to provide us with the essential information or have only provided us with inadequate information, we may not be able to confirm and process your registration application or to provide you with the requested information or services. If the documents you submitted to us contain any third party information, we will treat it as if you have already obtained the consent of such third party in disclosing such information and use it for the aforesaid purposes.
We will collect personal information from you, including but not limited to your name, gender, date of birth, contact information (including Hong Kong Identity Card No, mobile phone and phone number, email and address), together with information we have collected in respect of your previous transactions with Xcelom member platform and/or other associated companies ( If applicable) or the information generated when using our online services. Such information will accordingly be used in the scope of our business needs and to provide you with the services as set out hereinbelow.
Providing us with your personal information indicates that you have agreed to our relevant terms and conditions for collecting personal data and particularly collecting, using, transferring, storing and processing such data in a prescribed manner.
We hereby reserve absolute discretion to review, edit, change or amend this Statement, and also our exclusive rights to cancel or revise the policies and information listed on our website in accordance with the law without prior notice.

(i) Processing and administering the registration of testing appointments;
(ii) Processing and handling enquiries related to appointment registration;
(iii) Conducting COVID-19 tests and providing test results; and

In order to keep in touch with you and to keep you updated with our latest information, which includes our promotion and advertisement, collection of opinions, individual invitations, distribution of promotional materials and other various activities related to us, we will use your data to provide you with the aforesaid information. If you do not want to receive such information, you can notify us via email: email@example.com and we will stop using your personal data for the said purposes after processing your instructions.
We will use your information for direct marketing purposes only with your consent, and you have the right to choose not to accept relevant promotional information.
This Statement is also applicable to the personal information you have provided to us through any other form of offline communications (for example, filling in the application form to join any of our clubs, or using the registration form on the flyer to participate in special offers or promotional activities, etc.), and the personal information collection statement and membership instructions will also be interpreted and enforced accordingly.
Accessing and Amending Personal Data
If you want to inquire, amend or delete the personal information you have already submitted, please email to firstname.lastname@example.org. We may charge a reasonable fee based on the administrative and actual costs incurred by your request for data access.

We will keep the personal information provided by you in confidentiality in accordance with this Statement. Web Records/Small Text Files (Cookies)
We will use small text files (i.e. text data files containing user preferences, favorite web pages, etc.) to collect statistics about users' preferences for online services provided by our website. The application of small text files will not be used to collect any personal data. When you use this system for appointment registration, we may collect the IP address of the registrant for analysis to prevent any malicious attempts to interfere with system flow. These data will only be used to optimize the system. We will not use the data for identification purposes.
You will have to clarify and estimate the correct period of validity which may be required by the relevant regulations of your intended country/destination. We shall only be responsible for providing relevant document related to laboratory test result.
After you have completed the sample collection, you will have to preserve the samples in accordance with the guidance provided by us. If the samples were not preserved correctly, the test results thereof might be affected.
The test results are solely based on the samples that you have provided during sample collection.
